Helping vulnerable people access the COVID-19 Vaccine

The Disability Trust is working to ensure that vulnerable people in South Eastern NSW who are unable to leave their home, have access to their first or second dose of the COVID-19 Vaccine.

Working in partnership with COORDINARE, SE NSW PHN  we are welcoming enquiries from potential patients, their carers and other relevant organisations, before assessing medical and Health District referrals. For those who are eligible, The Disability Trust then liaises with the person, GP networks and vaccination providers to provide in-home COVID-19 vaccinations.

Vulnerable people are those over 12 years old who are:

  1. living with a disability (sensory, cognitive, intellectual, psychosocial and physical) including:
    * Individuals who are living by themselves in a home with or without family or carer support
    * People with a disability who live in congregate settings
  2. People with immune-compromising conditions (including people being treated for cancer), multiple comorbidities, chronic lung disease, chronic neurological conditions, and those with severe mental health conditions.
  3. Carers or others who may be living in the same household as a person described above if they require vaccination and consent.
We recently received this lovely message from one the people we were able to help. He was delighted to be able to access the service, as he explained.

“I phoned the Disability Trust a couple of weeks ago, having already received my first vaccination back in September. Unfortunately, just before my second jab was due, I had a fall and twisted my knee and was unable to leave my home at all.

“It was important get the second jab, as I have two new Grandsons and I haven’t been able to meet them for three months. The Trust secured me a home visit by an extremely friendly and professional Doctor who came all the way from Nowra to help out.

“One week to go now, and I will be able to get to meet my Grandsons. My thanks go to The Disability Trust for organising this vaccination, I appreciate your efforts”
